Important: Application for Educational Benefits

Each school year families are asked to complete an Application for Educational Benefits. This application helps the state identify which students qualify for educational benefits such as reduced lunch costs. The funding tied to the process also pays for the free breakfast program for all students. It is extremely beneficial to the school for each family, even if you do not think you will qualify for the benefits, to complete and submit this form. Please, if you have not already, consider completing the application which will be mailed to your home address. It can also be done online at www.schoolmealapp.com.
 
Paper Form: If you think your family would not qualify and you are using the paper form, you can complete the top of the attached written form with your child's name and date of birth (DOB). Check the box right below the name space that says "Household Income Does Not Qualify for Educational Benefits."
 
Online Form: If you think your family would not qualify and you are using the online form, you can complete the beginning info of the online form with your child's name and DOB. Check the box that says "Household Income Does Not Qualify for Educational Benefits."
 
Applications must be submitted by December 12, 2018 in order for Burroughs to not lose out on possible funding.
